Job Description Social Worker – Older Persons Mental Health Service – Burnie TAS Full time contract up until 31st March Community Setting Join a collaborative multidisciplinary team ACM Healthcare is working on behalf of Government Health Service in Tasmania providing Older persons community mental health care in the North West. Join a multidisciplinary team delivering person centred, recovery orientated and trauma informed care, in accordance with National Standards for Mental Health Services. RESPONSIBILITIES COULD INCLUDE: Information, support, and counselling for individuals and families. Assessment of mental health problems. Treatment advice and support in community and inpatient settings. Home visits as appropriate or required. Liaison and referrals with other health care professionals and service providers as needed. Community education and health promotion.
