We’re looking for passionate ITC Caseworkers to provide specialist case management for young people (Aged 12-18) living in residential care. You’ll work closely with a multidisciplinary team including therapeutic specialists, house managers, and carers to implement case plans and deliver holistic, individualised, evidence-based, and trauma-informed support that empowers each young person to achieve their goal and help provide clearer pathways to permanency. Key Responsibilities: Develop tailored assessment plans, and coordinate and monitor support services. Undertake case planning and ongoing management to help young people identify and achieve their goals. Provide specialised, trauma-informed care to support young people with complex needs, improving their safety, permanency, and wellbeing outcomes. Collaborate effectively with multidisciplinary teams, as well as significant others including family members, carers, and community supports. We are looking for someone with: Bachelor’s degree in Community Services, Social Work, or equivalent Minimum 2 years’ case management experience Experience working with young people, ideally in Residential and OOHC (Out of Home Care), child protection, or youth services.
