We are looking for an enthusiastic and kind person to join the St Vincent's Virtual & Home (Community) team providing quality care to clients in their own home, ensuring they can remain independent, supported and empowered within their own homes and communities. On a daily basis you will be: Delivering personal care and support with daily living activities in a domestic setting Working collaboratively with the Home Care team to get the best outcomes for individuals Providing social outings and travel/transport services Providing in home respite guided by a Care Plan Monitor and report on changes to client's health through appropriate protocols Building strong relationships with clients and their families Make a real difference to the lives of others.
