Excellent benefits including salary packaging North Richmond location Full time, fixed term contract covering LSL from 04/018/2025 - 31/10/2025 About the Role As an Aged Care Nurse Practitioner, you have advanced skill and knowledge to provide assessment and management of clients which includes referral to other health professionals prescribing of medications and ordering of diagnostic investigations. As an Aged Care Clinical Nurse Consultant, you will provide advanced skill and knowledge on the assessment and management of the older person, supporting care coordinators to deliver evidenced based interventions, and specialist support liaising with other health care professionals. You will provide person-centred, expert knowledge for eligible, high-risk clients living with chronic disease, comorbidities, complex aged care and other health and wellbeing needs. The role will also include Advanced nursing care skills in assessment and clinical intervention Provision of leadership and support to the Complex Care Service clinicians Contribute to the development and implementation of nursing best practice standards Support triage clients into the service, ensuring smooth transitions between community and in-patient care. Classifications for this position will range from the below dependent on qualification: Classifications for this position will be ZJ4 ($113,646.00 pro rata per annum) Nurse Practitioner NO1-NO2 (143,208.00 - 145,927.60 pro rata per annum) Your Contribution You will have Bachelor of Nursing Minimum 3 years' experience in relevant acute/community setting Strong skills in comprehensive assessment of the older person You have good knowledge of the community supports available for clients Current Victorian Driver's Licence What we Offer Salary Packaging - Increase your take home pay! The key aim of the CC/CD Service is to improve health outcomes for people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. CC/CD currently consists of multidisciplinary teams based at SVHM Fitzroy and at North Richmond Community Health Hub. Clinical service delivery is undertaken in the clients' own home and their local community.
