Location: Southern Tasmania Occupation: Nursing Position Type: Permanent, Part Time The Grosvenor and Collins Street General Practice is a well-established medical centre with two locations, Sandy Bay and Hobart CBD. They have a dynamic team of 15 doctors and two full time nurses. They are looking for an additional part-time or full time experienced Chronic Disease Management (CDM) nurse to join our busy and professional team. This role will see you working as their CDM nurse to support GPs and their patients in managing all health conditions and within the busy treatment room to cover holiday and sick leave. You have the option of working 6 hours per day 3 - 5 days a week. The Position The Grosvenor and Collins Street General Practice are excited to offer an opportunity for a Chronic Disease Management Nurse to join their team, working across both of their practice sites. This role is designed for an experienced nurse with a passion for patient care and chronic disease management. Key responsibilities will include the development and review of GPCDM plans, conducting health assessments, and managing the CVC and ATSI programmes for our patients. The role is self-directed, with flexibility in working hours to support work-life balance. Additionally, the successful candidate will be expected to provide cover in the Treatment Rooms when other members of the nursing team take leave, ensuring continuity of care across the practice. The Role Conduct health checks and assessments to identify those at high risk of chronic disease or hospital admission. Prepare chronic care management plans to help patients meet their health and wellbeing goals. Coach patients on health behaviour change. Work collaboratively with GPs to promote preventive health care. Clinical patient care - including acute care, wound care, skin cancer medicine, immunisations, ECG, ABI, and spirometry within our busy treatment room. Selection Criteria The successful candidate will have: AHPRA registration - Registered Nurses preferred. Ideally 1-2 years of working within CDM in general practice, preferred but not essential Knowledge of Best Practice - preferred, but not essential. Excellent communication skills The ability to establish and maintain collaborative working relationships with both patients and team members - GPs, the clinical team and support staff. Understanding of the diverse role and nature of primary care nursing, the ability to work autonomously while remaining aware of the scope of practice/limitations, and willingness to seek help where required. Previous experience is preferred in a General Practice treatment room, however not essential.
