Support Coordination Level 2 - What You’ll Be Doing You’ll deliver person-centered Support Coordination services including, but not limited to: Supporting participants to understand and implement their NDIS plans. Building participant capacity and independence. Linking participants with mainstream, community, and funded support. Developing short, medium, and long-term goals. Supporting crisis resolution and plan barriers. Facilitating service bookings, agreements & plan utilisation. Maintaining compliance with NDIS Practice Standards and Code of Conduct with our support and guidance. Specialist Support Coordination Level 3 - What You’ll Be Doing You’ll deliver person-centered Support Coordination services including, but not limited to: Crisis planning, prevention and intervention. Addressing complex barriers and risks. Navigating multi-agency environments. Coordinating supports for participants with high-risk or complex needs. Designer complex service plans. Ensuring providers respond appropriately to risk and complexity. Facilitating service agreements, bookings, reviews and NDIA reporting. Ensuring regulatory compliance and risk management with our support and guidance. What You Need To hit the ground running, you’ll need: Level 2: Qualifications/experience in disability, community services, mental health, or related fields; understanding of NDIS standards and Price Guide. Level 3: Tertiary qualification (Cert IV minimum) in mental health, social work, psychology, OT, or related field; experience with high-complexity participants; knowledge of restrictive practice rules. Both Levels: NDIS Worker Screening, WWVP/Blue Card (state based), First Aid Infection Control, NDIS Orientation & Induction modules, ABN & insurances, strong communication and organisational skills.
