Program Lead, Reablement, Wellness and Social Participation Working in the Community Aged Care team Fixed term full time position Not for profit salary packaging About the Opportunity You'll oversee our reablement, wellness and social participation programs, working directly with older adults to help them achieve their goals and stay engaged in their communities. Based in Melbourne, you'll collaborate with multidisciplinary teams to deliver person-centred care that respects individual needs and preferences.
