About the role The purpose of this position within the Community Transition Care Program is to apply occupational therapy skills and knowledge in the assessment and treatment of clients in the local geographical population to promote, maintain and rehabilitate functional ability and improve quality of life. The Community Transition Care Program operates seven days a week with opening hours - 0800 hours to 1630 hours including public holidays. Nursing services are rostered weekends and public holidays. Further extension of operating hours may occur based on client and hospital and health service need. About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values: * Demonstrates competent knowledge and skill to provide professional advice. * Deliver quality occupational therapy clinical services across a variety of caseloads in a Community Transition Care home-visiting model in accordance with prescribed professional and ethical standards. * Makes more complex clinical decisions and solves problems under the clinical practice supervision or professional guidance of a more experienced practitioner. * Provide case management services to Community Transition Care clients.
