The Community Clinical Nurse is required to support the Barcaldine Multipurpose Health Service (MPHS) in providing advanced level of clinical services to the Community and Commonwealth Home Support Programme (CHSP). Must be able to apply advanced, comprehensive knowledge and skills in community care, aged care and whilst providing education and development of staff, portfolio management and demonstrate the following: * Practices autonomously * Provides leadership in clinical decision making to give and/or coordinate care to recipients of healthcare services. * Applies critical thinking across all the domains. This position supports the Clinical Nurse Consultant and Community Home Coordinator in the provision of leadership and direction to nursing staff, direct care workers and utilises data and information to ensure that the service is organised for safety, and measures are in place to capture the standard of service delivery.
