About you: In your new role you will: * Deliver evidence-based interventions and treatments, ensuring the highest standards of care and safety for patients with chronic conditions. * Facilitate and coordinate care among multidisciplinary teams, ensuring seamless integration of services and effective patient management. * Provide education and support to patients, empowering them with knowledge and skills for effective self-management of chronic diseases. * Adhere to clinical governance frameworks, participate in quality improvement initiatives, and ensure compliance with regulatory standards. * Contribute to the strategic and operational planning for chronic disease services, including resource allocation and management.
