You'll support nurses from orientation through to advanced practice, strengthening clinical confidence and ensuring safe, evidence-based care across the SAGE-Frailty Unit. About us: The Sub-Acute Geriatric Evaluation Unit (SAGE) unit is located on the 5th floor of the Rockhampton Hospital. SAGE is a specialised unit with 24-hour service focus, with inpatients ranging from short term to longer term. The SAGE Unit aims are to provide coordinated interdisciplinary discharge planning, prevent premature admission to a residential facility and prevent readmission to hospital. About the role: The Clinical Facilitator is an advanced Practice Nurse who delivers a variety of clinical education activities designed to meet identified learning and development needs of Nurses. The Clinical Facilitator works primarily in the clinical area offering direct support and supervision to Nurses to promote and facilitate clinical care outcomes. This role will be required to work within the SAGE- Frailty Unit within the Hospital. About you: * You are a current AHPRA Registered Nurse with strong clinical capability in geriatric or rehabilitation care. * You bring advanced knowledge of teaching, learning and clinical development. * You enjoy supporting others and can guide nurses with warmth, clarity and confidence. * You communicate with empathy, respect and professionalism across teams and families. * You value evidence-based practice, continuous improvement and shared learning. * You thrive in a collaborative environment and are committed to exceptional patient care.
