Your Responsibilities: Build and maintain strong relationships with clients and their families. Ensure quality and compliance by conducting regular visits to clients' homes. Assess client needs, develop individualized care plans, and evaluate their effectiveness. Grow your client caseload through proactive business development Develop and sustain connections with internal and external stakeholders to deliver a range of services. What We’re Looking For: Experience: Demonstrated background in a similar Case Manager role (AHPRA registration is desired but not essential) Qualifications: Certificate IV in Aged Care or an equivalent qualification (preferred). Skills: Proven ability to establish and maintain strong community networks and relationships. Knowledge: Solid understanding of the Aged Care sector, including Support at Home, Home Care Package guidelines, and relevant service requirements.
