About you: In your new role you will * Be self-directed and show initiative under the guidance of the Team Leader and Aboriginal and Torres Strait Islander Health Worker Cluster Coordinator as Professional Lead. * Develop, deliver and evaluate primary health care programs for Aboriginal and Torres Strait Islander communities, ensuring programs are responsive and consistent with community needs, values and beliefs. * Actively participate in weekly multidisciplinary case conference and triage under direction of Registered Nurse / CNC.
