Your Opportunity A new position for a case manager has become available within the YouthBeat team in Northbridge. This role will attract a passionate Youth Work practitioner who has case management experience and a strong track record working in the youth, AOD or mental health space.
YouthBeat provides outreach case management and support for young people across the Perth Metro area experiencing risk factors impacting their lives. YouthBeat also works collaboratively with the WA Police, Department of Communities Crisis Care Unit and the Nyoongar Outreach Services to operate the Youth at Risk Strategy (YARS). YARS operates on Friday and Saturday nights to support young people under the age of 17 years in the CBD and Northbridge area by providing a safe youth-friendly place for them to wait while arranging their return to appropriate accommodation.
All young people engaged through YARS are also offered the opportunity to engage in YouthBeat case management and support. Your key responsibilities will be to: Receive and respond to referrals of new young people accessing the service Provide person-centred case management, and outreach to young people experiencing AOD and complex mental health issues Work with young people to develop holistic and strength-based case plans Prepare case information for, facilitate and attend multi-disciplinary clinical reviews Complete intake process and assessments for new young people accessing the service Requirements for Success To be successful in this role, you will need to possess: A Degree or Diploma in Social Welfare/Social Science/Youth Work or a minimum of three years' experience in a similar role. Experience and ability to work with at-risk young people experiencing alcohol and other drug use issues, mental health issues and mental health crisis Knowledge of child and adolescent development and child and youth safe practices.
