The role itself: Work and live in Albany; a bustling coastal town only 5 hours from Perth Option for part time (minimum 3 days per week) or full time Work across the Great Southern community, providing services to elderly home care clients Apply your specialist knowledge to empower clients, maximise their independence and improve their sense of well-being Develop, implement, evaluate and adjust comprehensive, holistic and progressive care plans Involve clients in the decision making process for their care Prescribe equipment and recommend home modifications Identify changes in conditions and effectively communicate with the multi-disciplinary team Deliver advice, education and training to clients and their families What makes this opportunity great?
