Supported Living Facilitator

We are currently looking for a Supported Living Facilitator to join our Team on a Casual basis within our Gatton location. The Role As a Supported Living Facilitator you can make a difference to the lives of the people we support under the NDIS. You will work with people who have varying abilities who may be impacted by physical and/or psychological disabilities to help ensure they are able to maintain quality of life.
The support we provide varies this could include but not be limited to helping the people we support in their own homes or within group care homes assisting them with tasks and activities such as day to day household and/or personal tasks (such as bathing)which could include overnight stays, getting out for a coffee or catch-up with friends, participating in a hobby or sporting activity, going on day trips or holidays, visiting medical appointments and any other support that may contribute to helping them achieve their goals while staying healthy, happy and safe. About the Person you will support We require an energetic, forward thinker, fun loving, very patient, understanding, passionate and very reliable person to support an active lady. Must have a passion for Rugby League and be willing to support to local games, home and away.
Other events or functions out in the community as well. Use of own vehicle with full comprehensive insurance maybe required. Communication on various levels is important.
The successful person will also be able to think on their feet and be willing to change focus spontaneously and be able to work well in a team as well as autonomously. About you You must be available to work weekends at least 12 hours (6 hours on a Saturday and 6 hours on a Sunday) and times will vary. Be available to cover shifts at a moments notice, and in addition leave cover.
You will be an active member of your local community, have passion, energy and a background that has seen you working or volunteering within a caring capacity role previously supporting people within all community groups including (Aged, Disability or Children & Youth). Although not mandatory, qualification within community or human services would be desirable. Before commencing with UCQ you will also need to provide a Senior First Aid Certificate, Drivers Licence and currently hold or be willing to obtain positive notice from Disability Services (Yellow Card) and Working with Children Check (Blue card).
