Employment Type : Permanent Full Time Position Classification : Registered Nurse Remuneration : $64,184 - $90,122 per annum Hours Per Week : 38 Requisition ID : REQ163781 Sydney Local Health District (SLHD) is seeking a Registered Nurse to join our team Community Older Peoples Mental Health based at Canterbury Hospital Do you have demonstrated competencies to provide contemporary mental health practice? Do you have understanding of and commitment to recovery oriented services and consumer involvement in service provision? The Community Mental Health / Older People's Mental Health Team model of care states: The "Community Older People's Mental Health Team" (OPMH Team) is a specialty service of the community mental health system. OPMH Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.
Consumers under the care of OPMH Teams will either have: 1. Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months 2. A period of treatment as recommended by evidence based practices.
The clinician will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description. To be Considered For This Role You Must Have The Following: Current nursing registration with the Australian Health Practitioners Regulation Agency (AHPRA).
