About the Position Centacare is a registered provider of support coordination under the National Disability Insurance Scheme (NDIS). Centacare’s Support Coordinators help strengthen participants' ability to coordinate supports and participate in the community.

Centacare offers Coordination of Supports and Specialist Support Coordination. Support coordination is aimed at helping NDIS participants to increase their capacity in implementing all components of their plan, including informal, mainstream, community and funded supports. Support coordinators work creatively and resourcefully with participants in how they utilise their support budgets to achieve their goals.

Support coordination enables participants to: maximise the value for money they receive from their supports genuinely exercise their choice and control implement their plan have increased capacity to manage/direct their own supports have greater opportunities to explore and connect with community and alternative support options better coordinate multiple supports and services have the capacity of their informal support network strengthened be better able to use the NDIS Participant Portal ‘myplace’ About You To be considered for this position you will need to meet the following criteria: Essential Criteria Degree Qualification in Social Work or other allied health qualification Valid Department for Human Services (DHS) Working With Children Check (WWCC) previously known as Child-Related Employment Clearance Valid DHS Disability Services Employment Check Valid Driver's Licence Desirable Criteria Experience working with individuals with disability or psychosocial disability and their networks Knowledge and experience working with the NDIS Our Culture and Benefits At Centacare we believe that everyone has the right to be treated with respect.
