Transition Care Program - Case Manager Part Time, Fixed Term Position for 12 months (Parental Leave replacement)64 hours per fortnight based at Royal Park Campus Assessment, care planning and discharge planning - Case Management The Royal Melbourne is one of Victoria’s largest hospitals providing a comprehensive range of health services including acute, sub-acute, general, and specialist medical services throughout inpatient and community based facilities. We are also a proud part of the world renowned Melbourne Biomedical Precinct, working in collaboration to advance research for continuous improvement of clinical care. Your Opportunity: Assess and manage all assigned clients according to TCP and MH policies and procedures Actively participate in client goal setting and care planning in consultation with clients and carers Work as part of a dynamic team and autonomously as required Meet relevant TCP guidelines, services targets and Key Performance Indicators About You: Knowledge of Community Services and Aged Care Strong assessment and discharge planning skills Highly developed organisational, planning and problem solving skills.
