This position is for a qualified Social Worker who identifies as an Aboriginal or Torres Strait Island and the purpose of the position is aligned to the SVHNS Aboriginal Health Plan and the St Vincent's Health Australia National Reconciliation Action Plan The successful Social Worker Level 1-2 is responsible for the provision of a comprehensive social work service to patients / clients and their families, liaison with other health care professionals within St Vincent's Hospital Sydney and with external agencies in the areas of clinical allocation. In this position based within Cancer Services you will be part of a team of Cancer Social Workers (Bone Marrow Transplant/ Haematology/ Medical Oncology/ Radiation Oncology/ Head and Neck Surgery/ Breast Cancer ) providing high level comprehensive social work service within a multi-disciplinary team Monday to Friday for inpatients and outpatients.
The position will involve working closely with the St Vincent's Hospital staff, community, government and non-government organizations and service providers. You will be working full time, providing consultation, advocacy, clinical assessments and counselling to patients, families and carers. The social worker will contribute to complex care and discharge planning for vulnerable patients.
The successful social worker shall provide clinical cover for colleagues as required. You will also be providing consultation to team members and education for patients and staff to enhance bio-psychosocial learning. Social Work in the Cancer stream will be a primary allocation but with a need for flexibility to work according to clinical demand and priority.
