gl/iShcmS What you'll be d oing The Nurse Practitioner is an expert practitioner who, through leadership, facilitates research based clinical practice aimed at providing health care to patients with complex needs within a defined scope of practice. Primary responsibilities include advanced clinical assessment using diagnostic capability, planning and engaging others, prescribing and implementing therapeutic interventions and evaluation of outcomes to improve practice. The nurse practitioner role includes assessment and management using nursing knowledge and skills.
The role may include, but is not limited to, the direct referral of patients to other healthcare professionals, prescribing medications and ordering diagnostic investigations. The role is grounded in the nursing profession's values, knowledge, theories and practice, and provides innovative and flexible health care delivery that complements other health care providers. In particular, the Nurse practitioner will facilitate the provision of co-ordinated and timely clinical care for people with Congestive Heart Failure, who are at high risk of hospitalisation, across health service sectors including primary, acute, and secondary and community services according to the Integrated Care Model.
The Nurse practitioner will also engage with the patient, carer, GP and primary care teams, and actively contribute to the development of patients' individualised and multidisciplinary care plans.
