An exciting opportunity exists for an experienced Registered Nurse with recent experience in managing patients with complex aged care/dementia issues, and with a strong interest in and commitment to supporting community nurses in managing patients with these conditions, to join the very experienced and dedicated Northern Sydney Home Nursing Service team, working across Northern Sydney Local Health District. 10 per week Hours Per Week : 38 Requisition ID : REQ163803 Where you'll be working This position is based at the Northern Sydney Home Nursing service, located on the campus at Macquarie Hospital at North Ryde, and covers all patients residing within Northern Sydney Local Health District. What you'll be doing The Clinical Nurse Consultant Aged Care/Dementia is an expert nursing practitioner and supports the educational and professional development of community nursing and allied health staff, patients, carers and families. In particular, one of the role's core functions is to support the community nursing staff in the provision of effective, safe and appropriate evidence based aged and dementia care, enhancing and extending their knowledge and skill base through the provision of expert assessment, consultancy, care planning and education.

A registered nurse who has a minimum of 5 years full time post graduate experience, with at least 3 years full time equivalent Aged Care/Dementia nursing experience.
In addition the applicant must have an approved post graduate qualification relevant to the speciality and hold a relevant Masters or is willing to enrol and commence a Masters in the relevant speciality field within two years of the appointment. Current NSW drivers licence. Highly developed interpersonal and communication skills (both oral and written) with demonstrated networks and partnerships with multiple / key stakeholders.
Demonstrated experience in utilising a range of electronic clinical and non-clinical software applications. Demonstrated experience in practice development and change management. Demonstrated ability to develop and provide clinical education and conduct research.
Demonstrated experience in the development and application of evidence based innovative practice, policy and procedure and clinical quality improvement strategies.
