Yorke & Northern Local Health Network, Port Pirie Indicative Total Remuneration: $79,954 - $83,765 pa - Temp F/T (up to 30 June 2022) - ASO4 Yorke and Northern Local Health Network (Y&N LHN) has moved to a Consumer Directed Care model for the provision of aged care, disability and home support services. This allows consumers to maintain greater control over their lives by allowing them to make choices about the services they receive and who delivers those services. In this role, you will be responsible for the coordination and provision of Home Care Package (Aged) service agreements to eligible clients living within the Yorke and Northern Region.

You will ensure that Home Care Package targets are met and that eligible clients receive appropriate services to assist with reaching their identified plan goals, and ensure that services offered are culturally appropriate and inclusive. You will monitor, promote and foster a culture centred on customer service, working independently to maintain program information and monitor budgets, produce reports and actively promote Country Health Connect as a provider. With prior experience in a supervisory role, you will have a proven track record in program planning, development, implementation and evaluation. This will include experience in developing budgets and monthly reports and meeting the service obligations of formal contracts and agreements.

You will bring the ability to communicate, consult, negotiate and advocate across a range of organisational levels, interacting cooperatively with professional staff and a wide range of health and service agencies.
