Join this Multi-Disciplinary Therapeutic team in the provision of therapy, trauma service, rehabilitation, assessment, reports and referrals. Services include the support of children (aged 0-6 years, 7-12 years) and adolescents (13-17 years) and adults (18 years ) at the clinic, at home and in the community. You will provide outreach services to remote communities every week/fortnight.
Your caseload will include: Therapeutic services to children, adolescents and adults with a range of needs (ASD, growing pains, muscular concerns, rehab, neurological conditions) Provide evidence-based physiotherapy interventions that that include prevention, promotion, remediation and compensation strategies. Review treatment plans at regular intervals through the application of outcome measures and physiotherapy assessments. Work within a trans-disciplinary approach in intervention in consultation with team members to meet identified child and family treatment goals.
Conduct sessions with other disciplines in a multidisciplinary or consultative model of practice as required. Compile Physiotherapy assessment and handover reports and any other documentation as required. This includes completion of therapy plans and summaries of sessions.
Complete documentation for client files including session by session record of client contact and correspondence. Work within a relationship-based service delivery model that incorporates active participation by parents/caregivers in sessions to achieve goals. Maintain a current developmentally orientated therapy plan that includes input from the family and other disciplines as required in a trans-disciplinary context.
Complete ongoing professional development in accordance with performance development plan to remain current and competent in clinical practice for this population. Provide case management within the key therapist role that includes liaison with key stakeholders. You will be eligible to apply for a government grant to cover relocation and living costs.
