About our organisation; Chorus was formed in 2017 by the merge of three established WA care providers, Care Options, Community First International and Volunteer Task Force. We are an Australian not-for-profit organisation providing services in Aged Care, Disability, Mental Health and Volunteering. Together our new organisation will deliver care and services to over 10,000 clients across metropolitan and regional WA with a 480-strong workforce and over 600 volunteers.

The Role: We are currently seeking applications for an enthusiastic Support Worker - Community Connections to join our team in Kwinana. The Support Worker - Community Connections is responsible for assisting customers to live as independently as possible by providing a range of supports in the community. Essential Criteria: Minimum Certificate III in Aged Care, Disability, Individual Support or other relevant qualification (or working towards obtaining qualification within 12 months) Relevant experience working with customers who are aged or with a disability; Demonstrated ability to work with people with diverse needs and preferences; Excellent communication skills both written and verbal to interact with a diverse range of customers, their families, carers, community agencies and other professionals; Enthusiastic and engaging with customer groups; Proven ability to provide support to a number of customers in a group setting; Confident in speaking to and directing groups of customers and volunteers; Understanding of professional boundaries and privacy and confidentiality issues; Ability to work as part of a team; Good computer & mobile device skills; A proactive approach to the safety and well being of yourself and customers; Willingness to travel to multiple locations throughout the Perth metro - starting your day in Kwinana and using our pool cars/ mini bus to get to our other sites; Comfortable driving a mini bus (can be driven on an ordinary licence).

Prerequisites: National Police Clearance of 6 months or less currency (or willingness to apply for and provide prior to commencing employment); Current Australian Drivers Licence with no restrictions; Comprehensive motor vehicle insurance and registration; Current First Aid Certificate (12 months or less currency) or willingness to apply; Working With Children Check (dependent on role if working with customers under the age of 18); What we will offer you: Attractive salary pa.
