Plan, coordinate and deliver high quality Occupational Therapy care to clients living in the community, by conducting comprehensive assessment to facilitate delivery of multi-disciplinary care, consistent with South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.
You will be providing Occupational Therapy specific interventions and Case Management of clients enrolled with TACP. The Level 3 Occupational Therapist is a senior member of the Team, and will provide supervision to the two Level 1 /2 Occupational Therapists working in TACP. Selection Criteria Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and AHPRA.
Extensive post graduate clinical experience in Community and or Aged Care Demonstrated ability to perform comprehensive assessments, case management and provide effective Occupational Therapy to older people Ability to provide in-depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff Demonstrated commitment to ongoing clinical and professional learning.
