About the Department This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning. SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Northcote.
This position will sit within HIP's multi-disciplinary ALERT Psychosocial team and will provide an outreach service to the Salvation Army 614 Hub at 69 Bourke St, Melbourne. The 614 Hub is a safe space for people experiencing homelessness, social poverty and isolation to receive support by way of meals, clothing and preliminary counselling for addiction and mental health issues. Through multiple on-site and outreach programs, Salvation Army workers help to create pathways out of clients' current state of marginalisation.
The Hub provides an opportunistic location, where a large cohort of homeless individuals gather daily, and trust in the support of the Salvation Army. About the Role An exciting opportunity currently exists for a Senior Mental Health Clinician to join the Health Independence Program (HIP)/ALERT on a Part Time, Fixed Term basis. You will contribute to the development of a specialist mental health role within a developing primary health clinic based at Salvation Army project 614.
Under the guidance of the HIP Psychosocial Stream Manager, the Mental Health clinician provides a high standard of clinical assessment, treatment and support to clients who are experiencing significant mental health difficulties who present to the 614 Hub. This position will also assist in the facilitation of referrals and support for people to access existing mental health services as required including area mental health services, mental health community support services and private services. In conjunction with the Clinic nurse development of pathways to streamline service access Classifications for this position will range from RPN Grade 3 or AHP Grade 3 ($91,000 per annum).
