About the Role Western Health is seeking talented and skilled Geriatricians to join their growing team.
We have several leave cover roles available in 2020: 1. Full-time LSL cover from March-August inclusive 2. Part-time LSL cover from March-June inclusive 3.
Part-time Mat leave cover from Feb-November inclusive. These last 2 roles may also be suitable for a full-timer to cover with flexible part-time/full-time work from Feb-November. Western Health provides clinical services to older people across multiple sites, and across the care continuum, from emergency, acute care, subacute and community services.
The Division of Aged Care and Subacute Services provides quality health care for patients across Acute and Subacute Aged care, Rehabilitation services, Palliative Care and Transition Care Services across four sites. Aged Care and Subacute Services play an integral role within the health care continuum, supporting patients to maximise their independence and functioning and, in doing so, minimise long-term health and community care needs. This diverse group of services is fundamental in promoting effective and seamless services across the care continuum.
To ensure that patients have appropriate and timely access to these services, the Division also provides a Subacute and Nonacute Assessment and Pathways (SNAP) service to facilitate pathways to the right care setting. The Geriatric Medicine Unit is operationally and professionally accountable to the Division and provides medical leadership in their area of specialty across the continuum of care. Services are provided across the following care settings: Inpatient services include - Acute Aged Care, Emergency Department assessment Service, Orthogeriatrics, Cardiogeriatrics, Geriatric Evaluation and Management, Transition Care, SNAP.
Community services include - Hospital Admission Reduction Program (HARP), Aged Care Liaison Service (ACLS) Specialist Clinics include - Continence, Falls, CDAMS, Wound Management, and Movement Disorders. The Unit has a strong focus on undergraduate and postgraduate education and promotes a close liaison with regional partners. The service fosters person centred care at all levels, is focused on innovation in service delivery, and is committed to providing safe, high quality care to our community.
