Support Co-ordinator

On behalf of our client Just Better Care. We are currently looking for a Support Coordinator who is passionate about delivering high quality service. As a Support Coordinator you will be responsible for a case load of clients with whom you will developed Support plans, Organise suitable supports and make linking in supports to the client.

The desired person will have experience in case management in community services such as disability, mental health or aged care and have a sound knowledge of the NDIS and local services. There may also be the opportunity to diversify to Aged Care case management. This role will be based in Horsham and will require the person to travel to other locations within the region.

You will also need a current Federal Police Check, WWCC and current First Aid Certificate at own cost or discussed with your jobactvie provider . This is a 12 month contract position. Applications close 5pm Friday 30th August 2019.
