THRIVE Remote Case Worker
Thrive is a new support program for Aboriginal housing and public housing clients, replacing the Support and Tenant Education Program (STEP). It will take a holistic approach to enhance people's lives by looking at the wider needs of the individual. It will have a stronger focus on early intervention, supporting people to identify and tackle problems before they become too great.
THRIVE will assist clients to manage their household by developing strategies to grow their capacity and resilience, help clients to navigate and link up with support services and programs across government & the private sector, and provide clients with long term sustainable support networks. The THRIVE Remote Case Worker is responsible for ensuring that collaborative, high quality, client focused case management services are provided to people who are currently referred tenants, potential tenants or identified as being in need of housing. Responsibilities: Assist clients to set goals and address Department of Communities expectations Work with clients to achieve goals and expectations Document client progress and provide updates Prepare reviews of cases for review meetings Promote education and progress through the program Attend training as required Support clients to maintain safety (particularly women and children escaping family violence) Support clients with future planning Assist in dealing with issues relating to housing or accommodation Support clients in their contact with Department of Communities Support clients in dealing with day to day issues and issues that may impact on their living independently Support clients with issues that may be impacting on their ability to maintain accommodation Support clients with issues arising in relation to transitional housing or direct tenancies Qualifications Relevant tertiary qualifications in social work (e.
