An exciting opportunity has opened up for a Nurse Practitioner position to provide the efficient and effective delivery of professional nursing care to patients, residents and clients in Cessnock, NSW. As a Nurse Practitioner, you will be working in a large facility providing both high and low care. This team works hard to provide an atmosphere where the residents are comfortable and can feel at home. Their state-of-the art facilities afford them the opportunity to grow their community-based services, particularly in the area of primary health care, whilst remaining committed to their acute and aged care services.
There is also potential to provide services in the community under HCP, CHSP and NDIS. Duties: Providing advanced clinical skills and assessment/intervention and consultation Developing a â€˜shared careâ€™ partnership with GPs, contributing to care plans and working collaboratively with GPOs to co-manage care recipients. Facilitates and coordinates regular case reviews and review processes to ensure care recipient needs are clearly identified and consistent with management plans in collaboration with multidisciplinary team.
The assessment of side effect management for the care recipient receiving interventions. Identify training and development needs in the care of clients and initiate strategies to meet those needs. Conduct quality audits and surveys as required and/or requested.
Work collaboratively with residential care and nursing staff, general practitioners and allied health professionals to identify and manage care recipients requiring complex nursing care planning and advanced clinical nursing care interventions(s) to avoid risk of hospitalisation. Identifies and adopts innovative clinical practice models e.g.
initiation and evaluation of new treatments, technologies and therapeutic interventions. Requirements: Current AHPRA registration & endorsed as a Nurse Practitioner. A commitment to the Nursing & Midwifery Board of Australia, Competencies and Codes of Ethics/Professional conduct.
To be considered for this position, you must have empathy towards the aged and frail, be self-motivated and hold a National Criminal History Check (also known as a police check) less than 6 months old. Minimum of 5 years full time post registration experience, with a minimum of three years full time within the aged or community care environment. Demonstrated ability to work autonomously and as part of a multidisciplinary team.
