LHD Nursing Recruitment Solutions is currently seeking a Care Coordinator with experience Chronic Disease Management for our client in Victoria. Based 2 hours from Melbourne this small country has a strong community team encompassing over 60 staff to help patients stay well within the community both pre-hospital and post hospital care. You will join a great team, deliver excellent care in the community, research within Chronic health within the community and have a strong understanding of HIP (Health Independence programs), PACP (Post-Acute Care Programs), HARP ( hospital admission risk program), SACS (Sub- acute care ) also experience working with orthopaedic, Cardiac or Pulmonary Rehabilitation programmes.
