The Community Home Care Services Team is looking to appoint an enthusiastic and reliable Support Workers to support the aged care programs within Uniting SA.
Located within the metropolitan areas of Adelaide, we require a competent professional who is experienced working within a human services team and enjoys working in a busy environment. List of key responsibilities: Delivering services for clients at home including supporting and guiding clients to complete identified tasks as well as completing agreed tasks on behalf of clients. Undertaking of domestic tasks and personal care including cooking, housework, showering assistance, etc.
Contributing to assessments of the clientâ€™s goals, interests and needs. Assisting the service coordinator to maintain appropriate communication with carers and family members nominated by the client. Maintaining accurate comprehensive case notes, and promptly report any changes in the consumersâ€™ health and well-being.
