About the Department The SECU Diversion and Complex Care program provides highly targeted, intensive, integrated clinical and psychosocial support to consumers of St Vincentâ€™s Mental Health with severe mental illness and complex needs. These complex needs may include recurring homelessness, co-existing substance use issues, cognitive difficulties, challenging or antisocial behaviour or patterns of offending behaviours and associated interaction with the criminal justice system. About the Role The position, under the guidance of the Manager of CCU/PARC and in collaboration with the Project Officer, Complex Care Senior Clinician and Enrolled Nurse, will work across clinical teams and ensure continuity of appropriate clinical and psychosocial care through the coordination of relevant internal and external service providers, as well as the provision of direct support to consumers receiving packages.
This position will focus on those consumers identified for SECU Diversion and Complex Care packages or complex care funding to provide highly targeted, intensive, integrated clinical and psychosocial support and care coordination. Duties Support in establishing and coordinate multi-disciplinary, inter-agency care teams which include the consumer and their carers Coordinate the development and implementation of complex care plans Provide direct support to consumers where gaps in service are identified and support consumers to engage with and maintain relationships with relevant service providers Assist in exploring opportunities to establish relationships with relevant service providers Qualifications, Skills and Experience Bachelor Degree in either Nursing, Occupational Therapy or Social Work or Clinical Masters in Psychology Current registration or eligibility for membership with the Australian Health Practitioner Regulation Agency (AHPRA) or the Australian Association of Social Workers (AASW) Demonstrated ability and knowledge of a recovery oriented, community based psychosocial approach promoting social connectedness and inclusion Demonstrated ability to engage with, and work collaboratively with people with severe mental illness and complex psychosocial needs.
