You will provide a high standard of care in accordance with the Strengths model under a recovery oriented framework working closely with the medical and multidisciplinary teams to achieve a positive outcome for all consumers and their families. You will be offered a range of learning opportunities to develop specialist skills and the opportunity to operate as part of an integrated Adult Area Mental Health Service. The classification range for this position is Social Worker Grade 2 (YC42 - YC45, $78,956 - $90,766 per annum) Duties Co-ordinate, oversee and monitor admission tools working toward discharge planning for consumers Liaise with community groups and relevant agencies to develop and review community networks which enhance continuity of care Provide an advanced social work service which includes consultation and education for consumers, carers and other health care professionals in the Acute Inpatient Service Supervise and provide training to social students and other non social work staff according to service policy, University Schools of Social Work and AASW requirements Conduct comprehensive assessments, including bio-psychosocial assessment and formulate and implement treatment plans Facilitate individual and group work focusing on interpersonal skills and strengths of consumers utilizing recovery principles Qualifications, Sk....
