Disability Care Coordinator



My client is seeking an individual who is passionate about the aged care industry that will be able to be a positive addition to the organisation for an immediate start located in the Northern suburbs. Client Details My client have an opportunity in their Disability Care Division, for a Disability Care Coordinator to work across their Supported Accommodation sites throughout the Perth Northern Suburbs. This role is responsible for the management of clients and staff and includes understanding individualised services and contemporary delivery of support.

The Coordinator will be directly responsible for developing and negotiating individual client’s budgets with funding bodies, developing and reviewing care and service plans, liaising with clients, their carers and guardians to ensure they are client directed. This is an amazing opportunity to work with an evolving not for profit organisation that values your future development and growth within the company Description Key duties for this role include: Contribute to identifying client and/or community needs Monitor program or client progress against contractual and/or funding requirements and consumer's needs Complete reports as required for funding/external stakeholders and Rise management Develop, monitor and actively manage the program, house or client budget addressing surpluses or overspends immediately Contribute to the development of the program’s budget In conjunction with the client and their carer / stakeholders, develop/review client plans and track progress Regularly review individual or program funding to determine if it is meeting the needs of the people we support and make applications for further funding (if appropriate) Develop, Implement and review client plans Monitor, measure and evaluate wellness outcomes and client goals Review, record and resolve complaints and compliments within organisational time frames Coordinate the recruitment, selection and development of staff Conduct job-specific inductions Monitor, review and manage staff performance Mentor trainees, support workers and volunteers Actively support and implement organisational change Maintain a professional and positive manner while role modelling relevant service standards to staff, volunteers and clients Maintain documentation as required by the organisation and funding body Ensure compliance with policies and procedures, highlight gaps and generate solutions Contribute to the monthly Divisional program reporting requirements Write accurate and detailed client/ support workers/volunteers file notes and ensure the quality of the team’s file notes, providing development as required Develop and maintain a positive and collaborative team environment within the workplace Enact and promote Rise’s values and ethos in all relationships and communication Demonstrate a commitment to quality, continuous improvement, occupational health and safety and risk management Maintain ongoing communication with Line Manager Man.
