Job Description About the organisation Australian Nursing Home Foundation (ANHF) is one of the largest Chinese aged care organisations in Australia. ANHF is a community-based, not-for-profit organisation founded with a vision to support and care for the elderly in Chinese and Southeast Asian community. About the role To provide a high quality transport service to frail elderly Chinese and South East Asian people, people living with dementia or other chronic conditions; and to their carers.

Service Planning and Delivery Assist TL/CIC/Coordinator in transport/outing planning, within budgetary constraints, to ensure the needs of clients and their carers are met. Provide client transport service for Centre based attendance, outings and/or special events in accordance with daily schedule. This includes pre-planning routes; undertaking pre- and post-transport bus/car safety checks; ensuring number of passengers does not exceed limit; communicating with clients/carers; safe and responsible driving; remaining with the vehicle at all times when clients are on it; assisting clients on/off the bus; and assisting with safe storage on the bus of clients' equipment and other personal items.

Exercise duty of care in day to day role; and observe and inform the TL/CIC/Coordinator of any noted change in clients' conditions or care needs. Ensure bus/car regular servicing is conducted, as per schedule, registration is maintained, and repairs and maintenance are attended in a timely manner for continuity of service to clients and safe operations. This includes driving vehicles to and from car service locations and heavy vehicle inspection sites.

Ensure bus/car is maintained with sufficient petrol levels and is cleaned regularly to be hygienic and comfortable for clients. Also assist with these tasks in relation to ANHF pool cars, if required. Report all bus/car damage and accidents/incidents immediately to TL/CIC/Coordinator, or more senior staff if unavailable, and actively assist with rectification under direction of TL/CIC/Coordinator.

Maintain accurate and adequate documentation as required, including bus/car driving log, pre- and post-trip bus/car inspections records, petrol and other expense receipts, and service/repair records. Liaise with Administrative Assistant regarding supplies and resources relating to client transport and ensure they are securely stored and are use economically (e.g.

first aid kit, spill kit, GPS and mobile phone). Contribute ideas and provide feedback in relation to the annual work plan, which is aligned with the ANHF Strategic Plan and Community Care Operational Plan. Service Evaluation and Improvement Provide accurate and timely data and information for input into management reports.

Assist with quality activities as directed and consistent with the ANHF approach e.g. KPIs, surveys, audits, reviews; and with preparations for external Quality Review.

Assist clients and carers to provide feedback about service delivery, including ad ho.
