About the Role Senior Clinician Occupational Therapist (Grade 3) ACE Team & Community HIP Outreach Temporary full-time role (Maternity leave until 08 December 2019) 7 day Rotating Roster including some weekend and evening shifts allowing for a flexible roster and the benefit of 5 weeks annual leave The successful applicant will be working in our busy Emergency Departments and in Community Services, supporting patient care via the delivery of a service model that is committed to the principles of Integrated Care to patients.
Western Health's - ACE (Advice Coordination & Expertise) Team is a multidisciplinary team providing care coordination services in the Emergency Department and inpatient wards for clients with complex medical issues at risk of presentation and admission. The role is to reduce presentations / admissions and optimise patient care through the provision of Care Coordination; Outreach services (Community HIP) and integrated care planning. The ACE Team and Community HIP Outreach Service is focused on building an integrated and supportive framework to strengthen and to promote person centred care.
This role gives the successful candidate the opportunity to expand their leadership and clinical skills in an innovative and supportive model. The role is primarily based in the emergency department, however will also include a component of outreach work; with both roles involving occupational therapy and transdisciplinary care coordination tasks. The Senior Clinician Occupational Therapy will also provide discipline specific clinical expertise to the other Community Services programs.
Professionally accountable to the Occupational Therapy Manager, this role will be part of the Occupational Therapy Senior Leadership Team and provide supervision to Occupational Therapy staff and students within the ACE and Community HIP Outreach team. Opportunities exist for the involvement in Occupational Therapy committees and professional development activities. The successful candidate will have commitment to developing integrated services for consumers with multiple and complex service needs that often 'fall between the gaps'.
