About the opportunity An exciting opportunity exists for a full-time permanent Senior Case Manager (Support Advisor) located at our Nowra office, providing support to the Illawarra/Shoalhaven area. Reporting directly to the Area Manager, you will be responsible for providing cost effective, evidence based and goal orientated case management while working in partnership with clients to achieve positive outcomes reflecting meaning and purpose for the client. Key responsibilities include; Leading and mentoring a team of Support Advisors Supporting the Area Manager in monitoring the standards of case management within the Home and Community Care Team Ensuring Uniting Home Care Packages (HCP) targets are achieved and maintained Enhancing the team's capacity to provide a high standard of practice, by fostering good working relationships and providing consultation, advice and case management support to Support Advisors Balancing self-management and consumer directed care with duty of care and organisational capacity to deliver services Understanding and applying changes to the aged care industry in an environment of rapid growth and diversity Qualifications Teritary qualifications in Aged Care/Social Welfare sector or equivalent experience Demonstrated case management and leadership experience in Aged care Case Management Society Australia and New Zealand membership Current First Aid Certificate Current Australian Full Drivers Licence and access to your own vehicle Skills and Experience Key to the success in this role is the demonstrated ability in the below skills and experience; Excellent written and verbal communications skills Ability to work autonomously and in a team environment Supervisory and team leadership experience Strong commercial skills and customer service focus Knowledge of Health or Community Aged Care Services or Home Care (desireable) Strong computer skills Excellent time management and problem solving Well-developed understanding and delivery of case management Post Graduate qualifications in Case Management or relevant field (desirable) Benefits & culture Uniting is proud to be an EEO employer who supports an inclusive approach in the workplace.
