Contribute to the vision of a new model of mental health care for consumers in Brisbane North. The Partners in Recovery (PiR) program in north Brisbane operates under the umbrella of the PHN (Primary Health Network) through a consortium of nine agencies. PiR aims to make a real difference to outcomes for people who experience severe and persistent mental illness.

A Full Time vacancy exist for a Coordinator with the Communify PiR team as a fixed term contract ending 30 June 2019. Communify is based in the inner north of Brisbane and has been providing support to vulnerable and marginalised people including people with mental health issues in the inner suburbs of Brisbane for the past 40 years. Communify brings knowledge and expertise in operating mental health services underpinned by a recovery oriented framework.

About the Role As Coordinator you must be ready to move away from the Case Work/Case Management approach by encompassing the bigger picture around care coordination, service integration and systems reform by: Working with people who experience severe and persistent mental illness and their carers, families, friends, service providers and community supports to facilitate access to the services and responses they need; Assist participants with their request to access services under the NDIS. This includes supporting them to gather evidence to support their application; Working collaboratively with a range of organisations and primary health care providers to coordinate, improve, change and integrate services; and Advocating for, and working on, system wide changes that improve consumer outcomes where existing responses are inadequate. To be successful Is this the right job for you? We are looking for someone who: Has a relevant tertiary qualification; Has a record of success in facilitating viable solutions for consumers with complex needs around case coordination, service integration and systems reform; Has excellent engagement and communication skills, together with experience working collaboratively to achieve good results for consumers and resolve systemic service delivery problems; Has a demonstrated high level ability to establish, develop and maintain multiple productive working relationships and networks focusing on better consumer outcomes through improving and changing the service system.

Demonstrates a recovery-focused approach, empathy, resilience and a capacity for assertive outreach with vulnerable and marginalised consumers; Report to the Team Leader always maintaining professionalism and attention to detail in preparing case notes, follow up and reporting; Is committed to open and transparent practice in a learning environment; and Has a proven ability to meet deadlines in a high demand work environment and to exercise judgment and initiative and be held accountable.
