Employment Type : Permanent Part Time (Job share may be considered) Position Classification : Registered Nurse Remuneration : $31.58 - $44.34 per hour Hours Per Week : 30 Requisition ID : REQ65130 Where you'll be working Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region.

What you'll be doing To facilitate the provision of co-ordinated and timely clinical care for people with Congestive Heart Failure, who are at high risk of hospitalisation, across health service sectors including primary, acute, and secondary and community services according to the Integrated Care Model. Engage with the patient, carer, GP and primary care teams, and actively contribute to the development of patients' individualised and multidisciplinary care plans under the supervision of the CNC & Nurse practitioner.
An exciting opportunity exists for a an experienced Cardiology nurse with an interest in Heart Failure to work with Heart Failure patients in an inpatient and outpatient setting as part of a NSW Health funded Integrated Care trial focusing on supporting primary care in Chronic illness. This established Heart failure service provides education and support in self management of Heart Failure through inpatient education, phone calls, home visits and nurse-led clinic appointments. This will be an opportunity to participate in improving patient outcomes and preventing unnecessary admissions.
