SA Coordinator of SupportItec Group
We currently have three, Full time, Permanent position available for a experienced Support Coordinator's to join our Adelaide office. Primary Responsibilities: The Support Coordinator’s primary role will be to ensure the smooth implementation of a participant’s NDIS plan, by identifying and meeting the needs outlined in each NDIS plan and managing any barriers which arise during the process. This role will also assist with providing support for additional or more complex needs such as psychological, mental health, and accessing health, education, or justice services.
More importantly, this role will represent participants to ensure that NDIS plan outcomes are achieved. Job Description Coordination of Supports includes: Understanding the plan Connecting with supports and services Designing support approaches Establishing Supports Targeted Support Coordination Reporting to the NDIA. We are looking for candidates with the following: Experience and understanding of the role of the NDIS Support Coordinator, or motivation to learn about the role Skills and/or qualifications in Social Work, Psychology, Occupational Therapy, other Allied Health (highly desirable) Excellent communication (written and verbal), consultation, negotiation, leadership and coaching skills.
Exceptional understanding of the NDIS market programmes. Proven experience working with people living with a disability/mental health Current and relevant experience with working with Australian Indigenous and CALD communities Excellent report writing skills Demonstrated ability to maintain and develop relationships with participants, families and service providers A natural forward planner who critically assesses own performance.
