Ongoing Full time position based at Dandenong. We are seeking a qualified and experienced Case Manager to join our friendly and supportive KEYS team at our Dandenong office. The Case Manager will oversee the care team of specialist professionals in delivering intensive case management for complex young people in the KEYS program.

The Position: The position objectives include; providing leadership and coordination of the care team for each young person placed in the KEYS program, providing support to young people in the KEYS program, and to lead a multi-disciplinary team to develop and implement trauma informed plans.
To Be Successful in this Position You Will Need: Tertiary qualifications, preferably in social work / psychology, youth work or equivalent. Commitment to providing quality care and coordinate the team of professionals involved to achieve positive outcomes. To have experience in leading multi-disciplinary teams.
