About the role As Client Services Manager/ Case Manager with our Home Care team in Blackburn South you will design Care Plans to enable our clients to enjoy life within their own surroundings for as long as possible. This is a part time role working 45.6 hours per fortnight.
You play a key role in enhancing the person's independence, providing them with opportunity to make their own choices, to maintain high self-esteem and to take their place and make meaningful contributions to their community. Key Responsibilities Develop person centred care plans; Coordinate all aspects of care delivery; Build positive relationships; Support and enhance team performance; Manage risk & maintain documentation requirements; Promote and encourage continuous improvement About you You are passionate and committed to supporting the aged and frail to maintain their well-being, dignity and independence, while adopting a practical and commercial approach to Care Planning and management.
