Position Profile: As part of a multidisciplinary team, the role plans, implements, coordinates and evaluates Social Work services to patients in the clinical specialty area of RITH using advanced Social Work clinical practice skills. Responsibilities include planning, implementing and evaluating patient safety and quality of care, in addition to supporting therapy assistant staff in Rehabilitation in the Home, coordinating duties and managing performance.
