Do your life's best work We are actively looking for a Clinical Nurse Consultant to help fill a 6 month fixed term position within our St Vincents, Darlinghurst clinic in NSW. As the Cancer Nurse Consultant, you are responsible for providing individualised and coordinated care for patients requiring radiotherapy in their cancer trajectory. The Clinical Nurse Consultant also provides expert clinical advice to patients, carers and other health care professionals and facilitates care management plans for patients who are referred to GenesisCare and who are discharged back to their health service provider.
This will be delivered by drawing on evidence-based nursing and supportive care interventions to respond to identified physical and/or psychosocial issues based on comprehensive clinical assessment. This role will also contribute to quality and best practice nursing standards and liaise closely with the clinical and operational staff at GenesisCare. What you will do: Create links and liaise closely with health services to ensure a seamless transition of patients between their primary health provider to GenesisCare St.
Vicent's Attend multidisciplinary meetings where required to remain informed and plan patient care Provide expert clinical advice to patients, carers, and other health care professionals Undertake a patient assessment to develop, facilitate and evaluate the care management plan for patients with complex health needs Provide education on complex clinical issues to patients and carers. Remain the point of contact for patients, staff and other health services involved in the patient care. Contributes to and develops evidence-based patient information Liaise, refer and communicate effectively with the primary health care team Provide leadership in the ongoing review of best practice and contributes to the development and delivery of nursing education Who you are: Registration as a Nurse with AHPRA At least five years' experience and demonstrated knowledge and expertise in the care of people with cancer Postgraduate Qualification in Oncology Nursing, Advanced Nursing, or a related field.
A commitment to ongoing professional development at master's Level Demonstrated ability to coordinate and plan the care of people with cancer across the continuum of care. Advanced interpersonal, communication and negotiation skills Demonstrated commitment to quality of patient care. A proven ability to work effectively in a multi-disciplinary team.
