Nurse Navigators work across system boundaries to actively improve health literacy in close partnership with patients and families and other health and allied health services to improve the patient's healthcare experience and quality of life.
The Nurse Navigator role aims to facilitate a patient's journey through an increasingly complex health system by: reducing fragmentation, mitigating barriers, educating and empowering others, and coordinating care.
