Aged Care Provider - Sydney 12 month fixed term contract Attractive remuneration car This leading aged care and welfare services provider is seeking an experienced Case Manager to join their NSW team Responsibilities: Caseload management, assessment and implementation of care plans Ensure compliance with relevant program guidelines, policies and procedures Case assessment and development of care plans, in consultation with family members Lead a team of Direct Care Workers; Develop, review and enhance management systems for Community Aged Care Services. Provide reporting, manage budgets and administration Provide advice and support to a wide range of stakeholders Successful criteria: Appropriate degree qualifications At least 5 years of experience working at a similar level within aged care community services Demonstrated knowledge/experience in the management and development of community care programs; Excellent communication skills, candidates with second language highly regarded Excellent reporting and budget management capability Advanced Microsoft Office skills Drivers licence, current police check and criminal record checks Available to commence a contract from July 2021 Interested applicants apply on line to find out more.
