Your new role As a Case Manager for Out of Home Care, you will support youth aged 0 – 18y/o who are placed in our of home care. The objective of case management will be to improve the quality of life of the youth by working from a framework of strengths based and person centred approaches.
You will complete comprehensive assessments including risk, referral and program engagement to help identify and meet the specific needs of each youth. It is essential that you identify and proactively manage issues that impact the youth’s wellbeing and take appropriate action to ensure risk is mitigated. You will also participate and contribute to the larger team on issues of service delivery, wellness and continuous improvement.
What you'll need to succeed Tertiary Qualifications in Social work/Science, Behavioural Science or Psychology 2 years post graduate experience as a case manager, with youth and families Out of Home Care (OOHC) experience is highly desirable Experience working with youth with complex trauma backgrounds Strong written and verbal communication and ability to manage relevant paperwork, report writing and case notes A well developed understanding of culturally and linguistically diverse client groups Ability to meet the physical demands of this role i.e. lifting children Driver's License Working With Children Check (WWCC) National Criminal Records Check Clearance What you'll get in return Immediate start Case Management role in Western Sydney Attractive hourly rate full time hours (37.
