Complex case management providing evidence informed clinical services, intake, risk assessment and referral. Your new company This highly regarded federal government service is responsible for the delivery of mental health services for clients and their families. Working as part of an Australian wide team who provide face to face counselling, telephone-based support and case management, you will be joining a large team of passionate and highly professional specialist staff, within a supportive working environment which provides clinical supervision, professional development opportunities and peer and management support Your new role As Clinical Care Coordinator, you will provide high-quality, evidence informed clinical services including intake, assessment and referral, complex case management and case coordination to a caseload of clients with identified complex and/or multiple needs.
Additional accountabilities include: Maintaining clinical records and collating and analyzing data from client records Preparing reports to assist in evaluating and improving service delivery Liaising and forming partnerships with a range of government departments, community based mental-health and welfare agencies Contributing to a shared knowledge base to inform appropriate practice and referral pathways Participating in community development and service promotion activities as required Contributing to the development and delivery of group programs as required What you'll need to succeed The successful candidate will possess qualifications in psychology, mental health social work or mental health nursing. Current registration with AHPRA (psychologists and mental health nurses) and AASW (social workers) is essential. Candidates will ideally possess extensive and relevant clinical and case management experience and a Working with Children (WWC) and/or Vulnerable People Checks/Clearance (or the ability to obtain).
